Patient Information First Visit
Your first appointment will be spent completing a Comprehensive Examination. This begins with taking all necessary radiographs (x-rays) and photographs of the teeth. Then your health and dental history will be reviewed followed by a detailed examination of your teeth and soft tissues. Finally the treatment planning phase will begin. You can expect to have all the necessary time to discuss your wishes and your treatment options. It is our goal to help you construct a long term treatment plan for your oral health and this process shouldn’t be rushed. If necessary a second consultation appointment to further discuss complex plans will be made.
For patients under the age of about sixteen the cleaning will likely also be performed at this appointment. For patients under the age of 18 a parent or guardian will need to be present for the exam.
We would ask that those patients taking medications please be prepared to provide a current list.
For transferring patients, copies of your treatment history specifically what kind of cleanings were performed as well as copies of your most recent x-rays will assist us in your assessment and planning.
Appointments
It is our goal to see you promptly at your scheduled appointment time. However due to the nature of dentistry situations do arise which can cause delay. We realize that your time is valuable and we assure you that every effort will be made to stay on schedule.
Your appointment time is reserved especially for you, and due to the increasing cost of operation and the increasing number of broken/failed appointments, a broken appointment fee of $50.00 per appointment hour will be charged for appointments broken with less than 24 business hours notice.
Business hours are 7AM Monday through 5PM Thursday.
We understand that events beyond anyone’s control occur but unfortunately we can no longer make any exceptions.
Financial Policy
At each visit we ask that you make full payment unless other arrangements have been made. If you have dental insurance we ask that you pay your estimated portion in full on the day of service.
Cash, Personal Check, Visa, Mastercard, Discover, American Express and Care Credit may be used for payment of your account.
Senior Citizens (65 years of age) will be given a 10% courtesy adjustment for any dental service provided.
Insurance
We accept most major insurances and will bill your insurance company for you. You are responsible for payment in full of all fees charged by this office regardless of the status of your insurance claim. While we accept most insurances, we are preferred providers for..
